About this demo and your data
What this is
A take-home assignment build for LexHive. It demonstrates a screening funnel, reliable lead capture, an automation layer and measurement plumbing. It is not a benefits service and is not affiliated with the Social Security Administration or any law firm.
What is collected
- Your location (state or territory), used only to show simulated service coverage.
- Coarse answers about benefits, application stage and work situation. No diagnosis, medication, income or documents.
- A name and email address, so a saved request can be identified. Nobody will contact you.
Unsent answers stay in your browser's memory only and are lost if you refresh. If you have already pressed Send, only a random submission identifier is kept in this tab so the result can be checked; no answers are stored in the browser.
What happens after Send
The request is saved to a database and given a reference. An automation then routes it to a review queue. In this demonstration that is where it ends: no email, call, referral or application is made. Test requests are deleted on a short schedule.
Advertising measurement
When enabled by configuration, a single "request saved" event may be reported to an advertising platform for measurement, with contact details hashed. Your screening answers are never included in that event. When measurement is not enabled, nothing is sent.
